How to Access Doctor, Dental, Labs and Radiology Benefits
  1. YOU MUST READ Important Instructions BEFORE visiting a doctor;


  2. Choose a provider from the website;

  3. Call the healthcare provider to make an appointment;

  4. Click here to print your Member Letter and present your letter and Member ID card at the time of your appointment;

  5. The provider will call the toll-free number on your Member ID card to verify your eligibility and to calculate the contracted rate for services provided; and,

  6. Pay the contracted rate in full at the time of service.

How to Access Savings on Prescriptions at a Local Pharmacy
  1. Choose a pharmacy in your area from the website;

  2. Present your Member ID Card. The pharmacist will need the instructions on the back of your Card to process your prescription through their online computer system and you will immediately receive your savings benefits rate; and,

  3. Pay your savings benefits rate when you receive your prescription(s).

How to Access Savings on Monthly Prescriptions through the Mail Order Pharmacy
  1. For drug pricing, click here or call toll-free 1-800-854-8764;

  2. Click here to print the Mail Order Form;

  3. Complete the requested information and enclose your prescription (be sure to include physician’s phone number);

  4. Sign and date the authorization form;

  5. Enclose your check or credit card information; and,

  6. Mail the form to the address on the form.
